Pontiac Power Train:clutch Assembly Questions

I have a 2004 pontiac montana. the transmission has been having trouble shifting from 1st to 2nd. It seems to miss. by that I mean when it goes to shift the rpms go up but does shift. If I let up on the gas for a few seconds it will shift. Like a

2000 Pontiac 5 speed manual transmission GRINDS WHEN SHIFTING from fourth to fifth.
At times the shift can be completed , other times not...any suggesstions.
