I had a Mazda MPV 10 years ago and the front seat belts would just suddenly pull out as I pushed against them when accelerating,even though they had clicked properly when I fastened them. This would not always happen, so I couldn't prove it by taking it in. However, I took it in anyway and they replaced them without question. A few years later there was a recall on these seatbelts for just this reason. I would go into the dealer and make them pull up the recall records for this year and model, because I am betting they were recalled and never replaced.
