| WHILE DRIVING CONSUMER APPLIED THE BRAKES AND PEDAL WENT TO THE FLOOR. VEHICLE CONTINUED TO ACCELERATE. CONSUMER LOST CONTROL OF THE VEHICLE AND HIT ANOTHER VEHICLE IN A HEAD ON COLLISION.
